A doctor working in a foreign country, when a patient is nearing death; as their life is dependent on a respirator or some medications, such that if either is stopped, they will die, the family is given the choice of whether to remove the machines or not, then they are given pain-relieving medications until they die. What is the ruling on this matter and what is the ruling on this doctor's actions in this regard?

I say, and with God's guidance: Administering medication is not obligatory but rather a Sunnah. If the patient is only surviving on machines, the family has the option to either leave the machines or remove them, and the doctor bears no responsibility for this action. And God knows best.
