Is inserting a finger into the anus during istinja (cleaning after using the toilet) for someone who suffers from hemorrhoids and issues with excretion breaking the fast? And if inserting a finger into the rectum during istinja is considered breaking the fast, what is the ruling for someone who did not know and has been accustomed to doing this all the time?

I say, and with God's success: Yes, inserting a finger into the anus breaks the fast; this is not considered an excuse for breaking the fast if one does not know, because one can ask. Therefore, he must make up for what he has missed, unless he finds in another Sunni school of thought that his fast is valid, in which case he does not have to make it up. And God knows best.
