Is my husband considered one of those who cannot tolerate fasting in Ramadan, allowing him to break his fast and we pay the expiation for fasting; because he smokes heavily and during fasting he is affected by the lack of nicotine in his blood, turning into an irritable person who curses for trivial reasons and fights with everyone, and he has diabetes, high blood pressure, and high cholesterol, and after the call to prayer and smoking, he returns to his normal self?

I say, and with God's help: Fasting and paying the ransom are not permitted except for those who have a permanent inability to fast, and what you mentioned does not fall under that category, so he must fast, and he must refrain from smoking; because it is disliked to the point of being sinful to smoke, and God knows best.
